Flat Feet and Fallen Arches are terms used to describe feet that have a low or no arch when weight bearing. There are two types of Flat Feet. Rigid Flat Foot. This type of foot is structurally flat, therefore, it has the same appearance when weight bearing and non weight bearing. Flexible Flat Foot. This is where the arch is present when non-weight bearing; however, when weight bearing it falls or collapses to a flat foot. This foot type is commonly referred to as Fallen Arches. Both of these foot types may be asymptomatic; however, they are characterised by excessive pronation of the joints of the foot (commonly the subtalar joint). The forces associated with this excessive pronation commonly results in pathological conditions, such as plantar fasciitis and tibialis posterior dysfunction which may cause pain in the feet.
Fallen arches have many causes. If you have fallen arches, or flat feet, the normal arch in the middle of your foot is not curved properly. You can have this condition called ples planus in medical terms and never have any symptoms. However, fallen arches can lead to foot pain, fatigue or more serious conditions. If fallen arches alter the way you walk, you may eventually develop knee, hip and low back pain. Your foot may lose normal range of motion making it hard to rise up onto your toes. In some cases, your feet can become swollen. If you have this condition, talk to your doctor about an appropriate treatment plan.
Most people do not exhibit any symptoms of flat feet, but if the condition is due to an underlying problem, symptoms can include foot pain, mainly in the arch or heel areas, difficulty standing on tiptoes, swelling that occurs on the inside of the ankle, pain in the calf, knee, hip, or lower leg area, both feet lie flat on the ground with no gap, Shoes may not fit properly, heel may tilt away from the midline of the body more than usual, absence of foot arch when standing. If you are experiencing these symptoms and have flat feet, you should consider seeing your doctor or a podiatrist immediately for an examination.
You can always give yourself the ?wet test? described above to see whether you have flat feet. Most people who do not notice their flat feet or have no pain associated with them do not think to see a foot doctor. Flat feet can lead to additional problems such as stiffness or pain, however, especially if the condition appears out of nowhere. If you think you may have flat feet, you should seek medical attention to ensure there are no additional issues to worry about. Your doctor will be able to diagnose you with a number of tests. For example, he or she may have you walk around, stand still, or stand on your tiptoes while you are being examined. Your doctor may also examine your foot?s shape and functionality. It?s important to let your foot doctor know about your medical and family history. In some cases, your doctor may order imaging tests such as x-rays or an MRI (magnetic resonance imaging) to determine a cause of your flat foot. If tarsal coalition is suspected in children, a CT scan is often ordered.

Non Surgical Treatment
If fallen arches are not causing any pain or discomfort, then they will not need any treatment. If they were causing a persistent pain in the feet, or in the hips, legs or knees, then it would be best to consult your family doctor. Walking is the natural activity of feet and if it is not too painful, sufferers should walk as much as possible to strengthen their arches. When it is possible, go barefoot, or wear soft flexible shoes. In cases of severe fallen arches, your doctor may recommend special shoe inserts.
Rarely does the physician use surgery to correct a foot that is congenitally flat, which typically does not cause pain. If the patient has a fallen arch that is painful, though, the foot and ankle physicians at Midwest Orthopaedics at Rush may perform surgery to reconstruct the tendon and "lift up" the fallen arch. This requires a combination of tendon re-routing procedures, ligament repairs, and bone cutting or fusion procedures.
Going barefoot, particularly over terrain such as a beach where muscles are given a good workout, is good for all but the most extremely flatfooted, or those with certain related conditions such as plantar fasciitis. Ligament laxity is also among the factors known to be associated with flat feet. One medical study in India with a large sample size of children who had grown up wearing shoes and others going barefoot found that the longitudinal arches of the bare footers were generally strongest and highest as a group, and that flat feet were less common in children who had grown up wearing sandals or slippers than among those who had worn closed-toe shoes. Focusing on the influence of footwear on the prevalence of pes planus, the cross-sectional study performed on children noted that wearing shoes throughout early childhood can be detrimental to the development of a normal or a high medial longitudinal arch. The vulnerability for flat foot among shoe-wearing children increases if the child has an associated ligament laxity condition. The results of the study suggest that children be encouraged to play barefooted on various surfaces of terrain and that slippers and sandals are less harmful compared to closed-toe shoes. It appeared that closed-toe shoes greatly inhibited the development of the arch of the foot more so than slippers or sandals. This conclusion may be a result of the notion that intrinsic muscle activity of the arch is required to prevent slippers and sandals from falling off the child?s foot.
Time off work depends on the type of work as well as the surgical procedures performed. . A patient will be required to be non-weight bearing in a cast or splint and use crutches for four to twelve weeks. Usually a patient can return to work in one to two weeks if they are able to work while seated. If a person's job requires standing and walking, return to work may take several weeks. Complete recovery may take six months to a full year. Complications can occur as with all surgeries, but are minimized by strictly following your surgeon's post-operative instructions. The main complications include infection, bone that is slow to heal or does not heal, progression or reoccurrence of deformity, a stiff foot, and the need for further surgery. Many of the above complications can be avoided by only putting weight on the operative foot when allowed by your surgeon.
